Smoke Detector & Carbon Monoxide Affidavit
This affidavit is required if you are refinancing or selling your home. The bank requests it as part of your title report. Please complete the form, with a notarized signature, and return it to our office. We do not need an original, so it can be mailed, faxed, or hand delivered.
Building Permit
The building permit is a two-page document. Please print on LEGAL sized paper. Both sides must be completed. Two copies must be submitted to the Building Department. Thank you.
